Responsibilities

• Assess patient condition and develop care plan.
• Coordinate care with physicians and interdisciplinary team.
• Educate patient and family on disease process and care plan.
• Ensure safety and documentation per Medicare guidelines.
• Observe and adjust plans as patient needs change.
