Alternate Solutions Home Health, LLC in Ohio seeks an experienced Registered Nurse Case Manager to lead patient assessment, care planning, and coordination across disciplines. You will supervise care, educate patients and families, and ensure compliance with regulations while maintaining high-quality, cost-effective care in patients' homes.

The RN will document thoroughly, communicate with physicians, and participate in on‑call and care conferences.

Qualifications

• Registered Nurse with current license in the state of employment.
• 1+ year RN experience in an acute care setting; home care experience preferred.
• Strong communication skills with patients, families, and physicians.
• Calm under pressure with compassionate, patient‑centered care.
• Ability to document accurately and follow regulatory guidelines.

Responsibilities

• Oversees the overall care of the patient from beginning to end of episode.
• Updates doctor orders in patient chart as needed.
• Notifies doctor of significant changes in patient condition.
• Coordinates multi‑disciplinary care and provides skilled nursing care.
• Administers medications as prescribed and supervises aides/LPNs as needed.
• Performs assessments to develop and adjust the plan of care.
• Maintains comprehensive clinical documentation per agency policies.
