Job summary

Home Care Network is seeking an experienced RN Case Manager / Recertification Nurse to provide patient care, manage admissions and recertifications, and lead a field team across North-Central Ohio. The role requires travel within a 1-hour radius, flexible scheduling, and a focus on high-quality home health services.

Ideal candidates have 1+ year in nursing with home health and case management experience, an Ohio RN license, and willingness to travel.

Qualifications

• Must be a graduate of an accredited School of Nursing.
• Active RN license in Ohio.
• Minimum 1 year nursing experience; home health preferred.
• Experience in case management and supervision preferred.
• Willingness to travel within service area and have valid driver's license.

Responsibilities

• Coordinate patient care as RN Case Manager and manage SOC, admissions, and recertifications.
• Supervise and coordinate RN/LPN/HHA team and oversee clinical workflows.
• Document and communicate changes in patient condition to care team; support charting and outcomes.
• Travel to surrounding counties to provide in-home care and perform visits.
• Support discharge planning, transitions of care, and ongoing quality improvement.
