Alternate Solutions Home Health of Columbus seeks a Registered Nurse Case Manager to lead patient assessments, care planning and coordination of multidisciplinary teams in the patient’s home. • Collaborates with multi-disciplinary teams to ensure quality care.

Responsabilités

• Oversees the overall care of the patient from beginning to end of episode.
• Updates all doctor orders in patient chart accordingly.
• Notifies doctor of any significant changes in patient condition.
• Manages multi-disciplinary care as applicable.
• Provides skilled nursing care in the patient’s home as ordered by physician.
• Performs OASIS assessments to develop an individualized plan of care.
• Administers medication as prescribed by physician.
• Helps decrease re-hospitalizations by front-loading visits for high-risk patients.
• Promotes continuity of care with admissions, transfers and discharges.
• Counsels patient and family on disease/injury processes and management.
• Supervises care provided by nurse aides and LPNs.
• Completes clinical documentation in accordance with agency protocol.
