Job summary

Alternate Solutions Health Network in Ohio is seeking a skilled Registered Nurse for home health care. The RN will oversee patient care from start to finish, update doctor orders, and coordinate with a multidisciplinary team to support in-home visits.

Qualifications

• Minimum of one year of RN experience in an acute care setting.
• Home care experience is desired.
• Excellent communication and patient-family interactions.
• Valid driver’s license and auto insurance.

Responsibilities

• Oversees the overall care of the patient from beginning to end of episode.
• Updates all doctor orders in patient chart accordingly.
• Notifies doctor of any significant changes of the patient’s condition.
• Provides skilled nursing care in a patient’s home as ordered by the attending physician.
• Completes clinical documentation in accordance with agency protocol and Medicare/Federal guidelines.
• Maintains active RN license.
