Alternate Solutions Health Network, LLC in Columbus, Ohio is seeking a Registered Nurse Case Manager (RNCM) to lead assessment, care planning, and coordination for patients in home health settings.

The RNCM educates patients and families, documents care in compliance with Medicare guidelines, and collaborates with physicians and a multidisciplinary team to ensure safe, effective, and cost-efficient care.

Competenze

• Minimum of one year of RN experience in an acute care setting.
• Home care experience is desired.
• Ability to effectively communicate with patients, families, physicians and coworkers.
• Detail-oriented with strong nursing judgment and documentation.

Mansioni

• Oversees overall patient care from start to end of episode.
• Updates doctor orders in patient chart.
• Notifies doctor of significant changes in patient condition.
• Provides skilled nursing care in patient’s home as ordered by the physician.
• Performs OASIS assessments to develop an individualized plan of care and makes adjustments as needs change.
• Maintains accurate clinical documentation and compliance with agency and Medicare guidelines.
• Coordinates multidisciplinary care under physician direction.
• Drives to patient locations and conducts scheduled visits.
