Job summary

Alternate Solutions Health Network is seeking a Registered Nurse Case Manager to lead patient assessments, plan of care, and coordination across disciplines in patients' homes. You will educate patients and families, ensure safety, and maintain compliant documentation under physician direction.

The role requires OASIS assessments, on-call rotation, and the ability to manage episodes from admission to discharge within a defined territory.

Qualifications

• Active RN license in the state of employment.
• Minimum of one year of RN experience; home care preferred.
• Ability to communicate clearly with patients, families and physicians.

Responsibilities

• Oversees patient care from start to end of episode.
• Updates orders and notifies physicians of significant changes.
• Coordinates multidisciplinary care and educates patients and families.
• Performs OASIS assessments and documents care plans.
• Delivers skilled nursing care in the patient’s home as ordered.
• Maintains accurate documentation per Medicare/federal guidelines.
