Job summary

Ohio Living is seeking a Registered Nurse Case Manager for Home Health to plan, coordinate, and deliver quality home health services in full alignment with agency standards.

You will assess patient needs, develop care plans, and collaborate with other health professionals to ensure safe, effective care in patients’ homes. Licensure in Ohio and at least 1 year of acute care or Med-Surg experience are required; driving and on-call shifts are part of the role.

Qualifications

• Current unencumbered Ohio RN license required.
• One year experience in acute care or medical-surgical nursing.
• Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint) and the Internet required.
• EMR experience preferred.

Responsibilities

• Plan, coordinate, and deliver quality home health care per company standards and regulations.
• Assess patient needs and develop a comprehensive plan of care.
• Communicate with other health care providers about changes in condition.
• Oversee implementation of the plan of care and evaluate effectiveness.
• Audits and maintains patient records per policies and regulatory guidelines.
• Provide education to patients and caregivers as needed.
• Assist with quality improvement projects as required.
