Job summary

Team Select Home Care seeks an experienced Registered Nurse Case Manager to oversee patient care plans and supervise staff in a home health setting in California. You will work under physician orders and the plan of care, coordinating between patients, families, and healthcare providers.

The role requires state RN licensure, at least 1 year of nursing experience (home health preferred), and strong communication, clinical assessment, and documentation skills.

Qualifications

• Registered nurse licensed in state(s) of practice.
• Must have nursing degree from an accredited program.
• Minimum 1 year of nursing experience; home health preferred.

Responsibilities

• Develops, delivers and revises individualized patient-centered care plans under physician supervision.
• Provides case management to an assigned group of patients and staff.
• Completes data collection sets and required documentation within timeframes.
• Performs initial evaluation visits and regular reassessments of nursing needs.
• Coordinates care among patients, families, and healthcare providers.
