Job summary

Team Select Home Care is seeking a qualified Case Manager, a registered nurse, to oversee the care of assigned patients and supervise staff per physician orders and the plan of care. You will report to the Clinical Supervisor in a home health setting.

You will develop and implement individualized care plans, coordinate care with families and external providers, manage documentation like OASIS and PAR, and ensure timely medication administration and patient education.

Qualifications

• Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing
• Minimum (1) year nursing experience required; prior home health experience preferred.

Responsibilities

• Develops, delivers/implements, revises, and evaluates individualized patient-centered care plans under physician supervision
• Provides case management to an assigned group of patients and ancillary staff
• Documents required data sets (OASIS, PAR) and other records within required timeframes
• Makes initial evaluation visits and reevaluates nursing needs
• Coordinates care between patients, families, the agency, and other healthcare providers
• Administers medications per agency policy and physician orders
• Participates in clinical record/utilization review and quality assurance activities
• Maintains confidentiality and ensures writing of patient instructions and care plans
