AccentCare in Michigan is seeking a full-time RN Case Manager to oversee care plans from admission through discharge, coordinating with staff nurses, LPNs, and CHHAs to deliver the plan of care.

This role combines direct and indirect patient care and requires at least 1 year of RN experience and CPR certification. Pay is per-point with an annual compensation estimate; schedule is M-F with on-call duties. Eligible for benefits including medical, dental, vision, 401(k) and more.

Qualifikationen

• Graduate from an approved nursing program and hold an RN license in the operating state.
• Minimum 1 year of RN experience.
• Current CPR certification required.

Aufgaben

• Assess nursing needs and manage the patient’s care plan.
• Complete initial admission assessment with patient history.
• Identify nursing diagnoses.
• Review medications for safety and interactions.
• Set realistic, measurable goals and coordinate interventions with the care team.
• Document assessments, progress and discharge notes promptly.
• Inform physician of changes in patient condition and needs.
