Job summary

AccentCare, Inc. is seeking a Registered Nurse Case Manager for Home Health in the Lancaster, PA area. This full-time role manages care plans from admission through discharge and coordinates direct and indirect patient care to ensure quality outcomes.

The ideal candidate has RN licensure, at least 1 year of RN experience, and CPR certification. Compensation is estimated between $87,000 and $100,000 per year, paid on a per-point basis with benefits described in the posting.

Qualifications

• RN license in the state of operation.
• Minimum of one year of RN experience.
• Current CPR certification.

Responsibilities

• Assess nursing needs and manage the patient's care plan from admission to discharge.
• Complete initial patient assessment upon admission, including patient history.
• Select appropriate nursing diagnoses.
• Assess medications for interactions, allergies, and adverse effects.
• Establish realistic, measurable goals aligned with diagnosis.
• Inform the physician of assessment results and interventions.
• Implement nursing interventions within the Nurse Practice Act and plan.
• Re-evaluate patients' clinical needs as required.
• Document and submit assessments, progress, and discharge notes in a timely fashion.
• Inform the physician and team of changes in the patient's condition.
