St. Luke’s Home Health is seeking a Registered Nurse to join the Home Health Team as a Case Manager in Boise, ID.
This direct care role involves traveling to patient homes from admission to discharge and collaborating with physicians and multidisciplinary teams to create individualized care plans. As a Home Health Case Manager, you will play a key role in collaborating with physicians and multidisciplinary team members to create and manage individualized care plans for a designated caseload. This is a direct care role, involving travel within Boise, ID and surrounding regions to deliver in‑home care from admission to discharge. Your work will ensure that each patient receives the highest standard of care, aligned with their needs and the care plan.
