Position Overview
As a Hospice Case Manager, you will play a key role in collaborating with physicians and multidisciplinary team members to create and manage individualized care plans for a designated caseload. This is a direct care role, involving travel within Boise, ID and surrounding regions to deliver in-home care from admission to discharge. Your work will ensure that each patient receives the highest standard of care, aligned with their needs and the care plan.
