Job summary

St. Luke’s Health System seeks a Home Health Case Manager to collaborate with physicians and the care team to plan and manage individualized in-home care for a designated caseload. This direct care role requires travel within Boise, ID and surrounding regions from admission to discharge.

The RN must be licensed in Idaho with BLS, and prior Home Health or Acute Care experience is preferred. Full-time hours include a 8:30 AM–5:00 PM schedule with on-call and weekend coverage as needed.

Qualifications

• Registered Nurse with current Idaho license.
• Basic Life Support (BLS) certification required.
• Home Health or Acute Care experience preferred.

Responsibilities

• Collaborate with physicians and the care team to develop individualized plans.
• Deliver in-home nursing care from admission to discharge.
• Coordinate travel within Boise and surrounding regions for patient visits.
• Support patient education and family training in the home setting.
• Participate in on-call rotations and after-hours coverage as needed.

Position Overview

What You Can Expect in This Role

• Full-time Schedule: Monday to Friday, 8:30 AM - 5:00 PM, with rotating evening on-call and weekend shifts

• Patient Population: Adults

• Manageable Caseloads: Daily visit requirements are below the state and national average

• Compensation: Paid hourly which includes drive time and mileage reimbursement for traveling within Boise and it's surrounding regions

• Additional Pay: On-call pay and applicable differential pay

• Bonuses: Sign-on bonus and relocation assistance available (where applicable)

Qualifications

• Education: Nursing Degree (ASN or BSN)

• Experience: 2 years of RN experience; Home Health or Acute Care experience preferred

• Licenses/Certifications: Current RN license in the state of Idaho; Basic Life Support (BLS) certification

• Other Requirements: Valid driver’s license and proof of auto insurance
