Job summary

AccentCare, Inc. seeks an RN Case Manager for home health coverage in Edina, Golden Valley, and surrounding areas. The role offers a $87,000–$97,000 annual pay range and a $5,000 sign-on bonus.

Schedule is Mon–Fri, 8:00 a.m. – 5:00 p.m., with pay on a per-point basis as part of an estimated annual total. The RN will oversee patient care plans from admission to discharge, coordinate with nurses and other clinicians, and ensure quality care delivery.

Qualifications

• Graduate from an approved school of professional nursing and currently licensed as an RN.
• Minimum of one year of RN experience.
• Current CPR Certification required.

Responsibilities

• Assess nursing needs and manage the patient care plan from admission to discharge.
• Complete initial patient assessment, including history.
• Identify appropriate nursing diagnoses and monitor medications.
• Set measurable goals aligned with diagnosis and inform physicians of results.
• Implement nursing interventions within the Nurse Practice Act and re-evaluate as needed.
• Document assessments, progress, discharge notes, and required paperwork timely.
• Communicate changes in patient condition to the physician and team.
