Job summary

Elevance Health is seeking an LTSS Service Coordinator – RN Clinician (Case Manager) to lead overall case management for members within licensure scope. The role emphasizes person-centered planning and care coordination across health and social services, with an emphasis on achieving optimal health outcomes.

The position is located in the Nashville area with hybrid work requiring 1–2 days in the office and flexibility for productive remote work.

Qualifications

• Active RN license (Ohio)
• 3+ years in case management or similar with chronic illnesses/disabilities
• Experience with telephonic and face-to-face assessments and care coordination
• Strong collaboration with medical directors and care team

Responsibilities

• Perform telephonic and in-person functional assessments to identify and manage member needs across health, behavioral health, social services and long-term supports.
• Coordinate and monitor individual care plans to optimize health outcomes.
• Manage care for members with chronic illnesses to ensure efficient utilization of benefits.
• Develop短 term and long term goals with members and families, identifying alternative care options as needed.
• Collaborate with physicians and interdisciplinary teams to support person-centered care plans.
