Job summary

Elevance Health seeks a LTSS Service Coordinator - RN Clinician in Nashville, TN. This hybrid role requires in-office presence 1-2 days per week to foster collaboration while maintaining flexibility for productivity.

Responsibilities include telephonic and face-to-face functional assessments, developing and monitoring individual care plans, coordinating with the health care team, and ensuring access to services within benefits. RN licensure and relevant case management experience preferred.

Qualifications

• Requires a high school diploma or GED and minimum of 3 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role.
• Current, active valid and unrestricted RN license in Ohio state(s) required.
• BA/BS in Health/Nursing preferred.

Responsibilities

• Perform telephonic and face-to-face functional assessments for the identification, evaluation, coordination and management of member's needs.
• Identify members for high risk complications and coordinate care with the member and health care team.
• Manage members with chronic illnesses to ensure cost-effective and efficient utilization of health benefits.
• Obtain thorough member history to develop an individual care plan.
• Establish short and long term goals with the member and caregivers, identifying needs for alternative levels of care or waiver programs.
• Develop the care plan for services for the member and ensure access to these services.
