Qualifications

• Two years minimum experience in serving individuals with chronic illnesses or disabilities in care coordination or similar role.

Responsibilities

• Assist RN in identifying members for high risk complications.
• Obtain clinical data as directed by the RN.
• Identify members who may benefit from alternative level of care or waiver programs.
• Provide collected information to the RN for care planning and monitoring.
• Coordinate care for members with chronic illnesses or disabilities with the health team.
• Apply clinical guidelines; disease knowledge used to inform decisions (not nursing judgment).
