Qualifications

Responsibilities

• Perform telephonic and face-to-face functional assessments to identify and manage members' needs.
• Identify high risk members and coordinate care with the health care team.
• Manage chronic illnesses to ensure efficient health benefits utilization.
• Develop individual care plans with member histories and goals.
• Collaborate with physicians and care team to implement care plans.
