Job summary

Elevance Health is seeking a LTSS Service Coordinator-Clinician in Nashville to work under the supervision of an RN, coordinating care for members with chronic illnesses and disabilities within licensure scope. This hybrid role requires 1–2 days in the office per week to foster collaboration while supporting productivity.

Ideal candidates hold an LPN/LVN or SW license, with 2+ years in a service coordination-like role; a BA/BS in Health/Nursing is preferred.

Qualifications

• Requires an LPN/LVN, LSW, LCSW, LMSW or license other than RN (as allowed by state law) and Nursing Diploma or AS in Nursing or a related field.
• Current, unrestricted LPN/LVN, LSW, LCSW, LMSW or license other than RN (as allowed by state law) in applicable state(s) required.
• Minimum 2 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or similar role.

Responsibilities

• Assists the responsible RN in identifying members for high risk complications and gathering clinical data.
• Identifies members that would benefit from alternative level of care or waiver programs.
• Provides information to the RN to verify, interpret, and develop/revise the member's care plan.
• Coordinates care for members with chronic illnesses in collaboration with the RN and health care team to ensure efficient use of benefits.
• Applies clinical guidelines and makes decisions based on member needs within scope of licensure.
