Job summary

PACE by IIH in Bakersfield, CA seeks an Experienced Registered Nurse Case Manager to coordinate health care services for an assigned panel of participants. Under the direct supervision of the RN Manager, you will assess, plan, implement, and monitor care, ensuring high-quality, patient-centered outcomes.

You will conduct home care assessments, supervise aides, document regulatory activities, and collaborate with the interdisciplinary team to optimize transitions between settings while supporting

Qualifications

• Graduate of an accredited nursing program.
• Current California RN license.
• CPR and First Aid certification.
• Minimum of three years health care experience, geriatrics.
• Minimum one year experience with frail/elderly population.
• Medically cleared for communicable diseases and has up-to-date immunizations.

Responsibilities

• Assessing participants’ health and needs and developing plans to improve.
• Conducting Home Care Nursing assessments to determine nursing and equipment needs in the home.
• Delivering and documenting home care nursing interventions including medications and tests.
• Provide regulatory assessments documentation for CMS PACE compliance.
• Provide on-site supervision to Personal Care Assistants and LVN in participants’ homes.
• Record participants’ progress, chart referrals, and schedule home visits.
• Coordinate participant care with outside providers and teams.
• Communicate with Community Liaison and after-hours on-call staff.
• Review medical records to ensure timely clinic documentation.
• Participate in staff meetings, trainings, and quality improvement initiatives.
