Innovative Integrated Health, Inc. in California is seeking a Registered Nurse Case Manager (RN CM) to oversee assessment, coordination, monitoring, and delivery of healthcare services for an assigned panel of participants in the PACE program.

You will perform home care nursing assessments, develop care plans with the IDT, document care interventions, supervise home health staff, and ensure regulatory compliance while coordinating with external providers to maintain continuity of care.

Qualifications

• Regulatory documentation and CMS/IDT compliance
• Patient education and counseling
• Phlebotomy and basic nursing procedures
• CPR and First Aid certification

Responsibilities

• Assess participants' physical and mental wellness and develop plans to improve outcomes.
• Perform Home Care Nursing assessments and participate in IDT meetings to plan care.
• Deliver and document nursing interventions, administer medications, and conduct tests as ordered.
• Document regulatory assessments per CMS PACE Manual requirements.
• Supervise Personal Care Assistants and LVNs in participants' homes as required.
• Track care hours, schedule home visits, and maintain records.
• Coordinate with external providers and ensure smooth transitions between care settings.
• Provide health education and counseling to participants and caregivers.
• Participate in end-of-life care coordination and support.
• Support enrollment efforts with Marketing and ensure continuity of care.
