Job summary

Innovative Integrated Health Inc invites applications for the Registered Nurse Case Manager position within PACE by IIH. You will assess, coordinate, and monitor home-based health services for an assigned panel of participants, ensuring high-quality, person-centered care in the home setting.

You will conduct home care nursing assessments, participate in IDT meetings, supervise Personal Care Assistants and LVNs, and document care plans in alignment with CMS requirements.

Qualifications

• Graduate of accredited nursing program.
• Current California RN license.
• CPR certification with First Aid.
• BSN preferred.
• Minimum 3 years health care experience with emphasis in geriatrics.
• Experience with frail or elderly population.

Responsibilities

• Assess participants' physical and mental wellness and develop care plans.
• Conduct Home Care Nursing assessments and participate in IDT meetings.
• Provide home nursing interventions and document care per CMS guidelines.
• Supervise Personal Care Assistants and LVNs in participants' homes.
• Coordinate care transitions and communicate with PCPs.
