Job summary

CarePartners is seeking a Home Care Registered Nurse Coordinator for PACE (Program of All Inclusive Care for the Elderly) to ensure high-quality, efficient in-home care for participants and families. You will coordinate day-to-day operations, work with a team of nurses, aides, and schedulers, and use telehealth and community resources to support health and wellness at home.

The role requires a BSN (or willingness to obtain BSN within 6 years), NC RN licensure, a valid driver’s license, and

Qualifications

• RN license in NC with current licensure.
• Compact license may apply; valid driver’s license with insurance proof.
• Two years of interdisciplinary team experience in hospital, nursing home, or community setting.

Responsibilities

• Coordinate day-to-day in-home services for PACE participants.
• Act as liaison between the home health agency and the PACE program.
• Coordinate nurses, home health aides, non-skilled aides, and schedulers.
• Assess participants in their home and coordinate plans of care with resources.
