Job summary

Universal Healthcare IPA, Inc. is seeking an RN Case Manager in Bakersfield, CA, onsite.

You will provide advanced case management across the PHM continuum, collaborate with the interdisciplinary care team, sign off on individualized care plans, monitor interventions, and support member engagement in self-management goals. The position requires an active California RN license, 2 years of nursing experience with care coordination, and the ability to travel as needed.

Qualifications

• Active California RN license required.
• Minimum 2 years nursing experience; case management or population health preferred.
• BSN preferred.
• Valid driver’s license and reliable transportation.

Responsibilities

• Manage a caseload of PHM members, including complex/high-risk cases.
• Conduct comprehensive nursing assessments addressing medical, behavioral health, and social needs.
• Develop, approve, and sign off on ICPs with input from members and care team.
• Reassess at appropriate intervals; coordinate with LVNs for plan accuracy.
• Maintain accurate documentation in the EHR and ensure timely updates.
