Required ADN with hospital experience; 1 year in Case Management or Utilization/ Home Care; NJ RN license; BLS. BSN is preferred; strong communication and organizational skills, EMR knowledge, and orientation completion are

Qualifications

• ADN required.
• 2–3 years hospital experience.
• At least 1 year in Case Management, Utilization Management, or Home Care.
• Completion of all Orientation Programs.
• Strong communication and organizational skills.
• Proficient computer skills.
• Strong critical thinking and assessment skills.
• Knowledge of EMR systems.

Responsibilities

• Provide nursing care within Nurse Practice Act and regulatory standards.
• Ensure patient safety and apply evidence-based practice.
• Report safety issues and medical errors.
• Know the patient population served.
• Supervise delegation and collaborate with care team.
• Set and evaluate priorities with measurable outcomes.
• Lead the interdisciplinary team and manage the care plan.
