Qualifications
- BSN or MSW degree required; RN with master's accepted.
- Current NJ RN or SW license required; CCM/ACM preferred.
- Minimum 6 years clinical hospital/home health experience; 4 years acute care case management; 3 years supervisory.
Responsibilities
- Provide strategic leadership for Case Management operations aligning with system goals.
- Lead workforce development, succession planning, and performance improvement initiatives.
- Oversee staffing models, workflows, and resource allocation to improve care delivery.
- Ensure regulatory compliance and standards across the department.
- Partner with physicians and leadership to drive care coordination and system effectiveness.
