Job summary

The position requires a Bachelor's degree (Master's preferred) and CCM certification is a plus. Expect 40-hour weeks with two evenings until 9pm.

Qualifications

• Active clinical licensure (RN, LCSW, LPC, or LMFT) required.
• Case management experience in a managed care setting.
• Strong customer service orientation and problem solving.
• Excellent telephonic communication and motivational interviewing skills.
• Highly organized and able to work independently; manage time effectively.
• Willing to work two evenings weekly until 9pm local time.

Responsibilities

• Coordinate all case management activities with members to evaluate medical needs and wellness.
• Develop strategies to address issues to improve outcomes and opportunities for wellness.
• Instruct programs and procedures in compliance with network management and clinical coverage policies.
