Job summary

CVS Health in Louisiana is seeking a full-time Care Management professional to coordinate care, support members, and educate on health management. The role involves collaborating with case managers, medical directors, and health programs to overcome barriers and achieve optimal outcomes.

You will apply motivational interviewing and coaching to empower members, document thoroughly, and ensure regulatory compliance while traveling within the assigned region as needed.

Qualifications

• Degree in Health Care/Human Services related field required.
• Case Management experience required.
• Excellent communication and telephonic skills; customer service oriented.

Responsibilities

• Consults with case managers, supervisors, medical directors and/or other health programs using a holistic approach to overcome barriers to goals.
• Presents cases at case conferences to obtain multidisciplinary review for optimum outcomes.
• Identifies and escalates quality of care issues through established channels.
• Demonstrates negotiation skills to secure appropriate options and services for member benefits/needs.
• Delivers motivational interviewing to ensure maximum member engagement and promote lifestyle changes.
• Provides coaching and support to empower the member to make independent medical and healthy lifestyle choices.
• Assists in encouraging members to actively participate with their provider in decision-making.
• Conducts comprehensive evaluations of referred members’ needs/eligibility using care management tools and recommends a case resolution approach.
