Job summary

CVS Health is seeking a RN-licensed Case Manager in Louisiana to coordinate case management activities and promote member wellness. You will design strategies to address outcomes and opportunities, integrating care plans with network policies and clinical coverage guidelines.

This full-time role requires strong communication, motivational interviewing, and documentation skills, with the ability to travel up to 125 miles and work a flexible schedule until 9pm on some days.

Qualifications

• Must reside in Louisiana, Regions-4 parishes listed.
• Current/Active RN licensure required.
• Case management experience.
• Strong customer service orientation and problem solving.
• Excellent communication and telephonic skills.
• Excellent motivational interviewing skills and ability to build rapport remotely.
• Excellence in documentation and regulatory compliance.
• Highly organized and self-driven with minimal supervision.
• Ability to travel within 125 miles for 1 day in the field.
• Variable schedule with up to 2 days weekly until 9pm local time.
