Job summary

This full-time role requires travel within a 125-mile radius, strong communication and motivational interviewing skills, CCM certification preferred, and eligibility for CVS Health benefits including medical, dental, and retirement plans.

Qualifications

• Degree in Health Care/Human Services related field.
• CCM certification preferred.
• Strong communication and problem solving skills.

Responsibilities

• Consults with case managers, supervisors, medical directors to overcome barriers to goals.
• Presents cases at conferences for multidisciplinary review.
• Identifies and escalates quality of care issues.
• Demonstrates negotiation to secure appropriate options and services.
• Delivers motivational interviewing to engage members and promote health changes.
• Provides coaching and support for independent health choices.
• Encourages member participation with providers in decision-making.
• Conducts comprehensive evaluations using care management tools and recommends approach.
