Job summary

CVS Health in Louisiana seeks a full-time Case Manager to coordinate care, educate members, and support disease management programs. You will work with care teams, identify barriers, and apply evidence-based practices to improve health outcomes. Travel within the region up to 125 miles and flexible scheduling are required.

Strong communication and motivational interviewing skills, plus a health care or human services degree, are essential. CCM certification is a plus.

Qualifications

• Degree in Health Care/Human Services related field.
• Case Management experience.
• Strong customer service orientation and problem solving skills.
• Excellent communication/telephonic skills.
• Willingness to travel up to 125 miles round trip for 1 day in the field.

Responsibilities

• Consults with case managers, supervisors, medical directors and/or other health programs using a holistic approach to overcome barriers to goals.
• Presents cases at case conferences for multidisciplinary review and optimal outcomes.
• Identifies and escalates quality of care issues.
• Demonstrates negotiation skills to secure appropriate options and services.
• Delivers influencing/motivational interviewing to engage members and promote healthy changes.
• Provides coaching and support to empower members to make informed health choices.
• Encourages members to participate with their provider in decision-making.
• Conducts comprehensive evaluations of referred members' needs/eligibility and recommends a course of action.
