Qualifications

• Active clinical licensure (RN, LCSW, LPC, LMFT) required.
• Case management experience required.
• Strong customer service orientation and problem solving.
• Excellent communication and telephonic skills.
• Motivational interviewing skills with ability to build rapport telephonically.
• Excellent documentation and regulatory compliance.
• Highly organized and self-driven with limited supervision.
• Effective time management and meeting performance metrics.

• Ability to work 2 evenings weekly until 9pm local time.

Responsibilities

• Coordinate all case management activities with members to evaluate medical needs and facilitate overall wellness.
• Develop strategy to address issues to outcomes and opportunities to enhance member's wellness through integration.
• Instruct programs and procedures in compliance with network management and clinical coverage policies.
