Qualifications

• Must be a graduate of an accredited school of nursing.
• Minimum three years in varied clinical settings; two years in Utilization Review/Management or Case Management preferred.
• Knowledge of social and physical factors affecting discharge and of community resources.
• Proficiency in word processing, spreadsheets, and data management.

Responsibilities

• Establish a safe, individualized discharge plan and proficient timely utilization management to maximize reimbursement.
• Collaborate with patient, family and healthcare team to achieve quality outcomes.
