Job summary

The Valley Health System in Southern Nevada is seeking a registered nurse to lead safe, individualized discharge planning and provide proficient utilization management to maximize reimbursement for patient visits.

You will collaborate closely with patients, families and the health care team to coordinate post-discharge care, address social needs, and ensure smooth transitions across VHS facilities with opportunities for professional development.

Qualifications

• Graduate of an accredited school of nursing.
• Minimum three years of varied clinical experience.
• Two years in Utilization Review, Utilization Management or Case Management preferred.
• Knowledge of social and physical factors affecting discharge and community resources.

Responsibilities

• Establish safe, individualized discharge planning and provide proficient timely utilization management to maximize reimbursement.
• Collaborate with patient, family and healthcare team to meet post-discharge needs.
