Job summary

Qualifications

• Active Rhode Island RN license required.
• Bachelor's degree in nursing required.
• Case Management certification preferred.
• AHA BLS provider certification required.
• Strong analytical and interpersonal skills expected.

Responsibilities

• Collaborate with medical staff to coordinate care and discharge planning.
• Develop initial discharge plan and reassess daily with the care team.
• Proactively builds post-hospital referrals with Transition Care Coordinator.
• Communicate discharge plans to patients and families.
• Document discharge disposition and patient handoffs across care areas.
• Support managed care strategies to reduce readmission risk.
