Job summary

St. Charles Health System is seeking an RN Case Manager to provide clinically based case management, overseeing utilization management and discharge planning for a defined caseload. The role collaborates with the care team to ensure efficient resource use and high-quality patient outcomes throughout the hospital stay.

The RN Case Manager will document care, participate in quality improvement, and support a healing environment across St. Charles sites as needed.

Qualifications

• Graduate of an accredited school of nursing.
• BSN preferred.
• Current Oregon RN License.
• AHA Basic Life Support for Healthcare Provider certification.
• 3 years of clinical nursing experience in an acute care setting.
• Knowledge of Quality Improvement.
• Experience in case management and/or discharge planning preferred.

Responsibilities

• Provide clinically based case management to support effective and efficient patient care.
• Manage utilization management and discharge planning for assigned caseload.
• Collaborate with care team to optimize resource use and patient outcomes.
• Lead discharge planning activities and coordinate referrals for post-acute care.
• Document care activities in the patient record and ensure regulatory compliance.
