St. Clair Hospital is seeking an RN Care Manager to ensure effective patient care delivery through collaboration with the multidisciplinary team. You will support discharge planning, care coordination, and utilization reviews across admission and post-acute care processes.

The role involves communicating with patients, physicians, and payors, coordinating referrals to community resources, and navigating EMR and web-based systems to optimize patient outcomes.

Qualifications

• Current licensure as a Registered Nurse in Pennsylvania.
• Two years of work experience as a Registered Nurse in an acute care setting.
• Highly effective communication skills.
• Strong negotiation skills and the ability to identify and address complex clinical and social issues with patient/family and members of the multidisciplinary care team.
• Ability to navigate electronic medical records and web based applications.

Responsabilités

• Coordinate discharge planning and care coordination with the care team.
• Perform admission, outpatient, pre-admission and continued stay utilization reviews.
• Complete referrals to community resources.
• Gather and communicate information to patients, physicians, and payors.
