Job summary

Qualifications

• Graduate of an accredited school of nursing.
• Current Oregon RN License; BLS certification.
• 3 years of acute care nursing experience.
• Preferred: Case management/discharge planning experience.
• Quality improvement knowledge.
• Preferred: Certification in Case Management.

Responsibilities

• Uses the nursing process to assess patient condition and outcomes.
• Confirms medical necessity for continued stay and creates discharge plans.
• Collaborates with the care team to educate patients and families for discharge.
• Documents care in the patient record per SCHC policies.
• Leads and participates in quality improvement and care coordination.
