Job summary

Advocate Health is seeking an experienced Registered Nurse Case Manager in Monroe, NC to manage patient care plans and discharge planning through collaborative, regulatory-compliant processes.

You'll work 40 hours weekly, Monday through Friday, 8:00 am to 4:30 pm, with benefits eligibility and a pay range of $38.20–$57.30 per hour. Apply your clinical expertise to coordinate services across settings and support patient outcomes.

Qualifications

• Graduate of a Board of Nursing approved nursing education program (RN).
• Current RN license in the state of work or NP license if applicable.
• BSN preferred; licensure requirements must be met for home state if applicable.
• Typically requires 3 years of related nursing experience and case management background preferred.

Responsibilities

• Conducts complete assessments and planning within timelines for discharge planning and care transitions.
• Provide continuity of care and discharge planning with regulatory compliance and timely communication.
• Initiate referrals and document discharge planning interventions and utilization reviews.
• Collaborate with healthcare teams to optimize care and remove barriers to discharge.
• Educate patients and families on access to services and care coordination.
