Job summary

Advocate Health is seeking a Registered Nurse for full-time home health in the Milwaukee area. The role involves initial assessments, care planning, and delivering direct patient care in specialty areas under physician orders.

You will update plans, consult with family and team members, and ensure documentation accuracy, including OASIS and 485 forms, with on-call responsibilities as needed.

Qualifications

• RN license issued by the state where practicing (Wisconsin).
• Basic Life Support (BLS) for Healthcare Providers within 6 months of hire.
• Valid driver’s license and access to a registered, insured vehicle.

Responsibilities

• Assesses patients and develops plans of care aligned with physician orders.
• Delivers direct patient care and adjusts nursing care as needed.
• Documents visit assessments, OASIS, 485 and verbal orders accurately.
• Communicates with physicians and health team members for care updates.
• Participates in discharge planning and quality improvement activities.
• Maintains accurate clinical records per agency, state, and federal guidelines.
