Responsibilities

• Deliver termination of benefits letters as determined by third party payers, ensuring understanding of rights and obligations.
• Ensure family/significant others understand treatment plans when patient cannot.
• Develop individualized care plans with patients and relevant professionals.
• Maintain accurate records of interactions, assessments, and service plans.
• Provide patients and families with information about services and resources.
• Monitor patient progress and adjust discharge plans as needed.
• Collaborate with multidisciplinary teams to ensure comprehensive care.
