Responsibilities

• Advocate for patients' needs and rights within healthcare and social services systems.
• Conduct comprehensive assessments to determine patient needs, healthcare goals and eligibility for services.
• Coordinate services and referrals to healthcare providers, community resources, and support services.
• Delivers termination of benefits letters to patients and ensures understanding of rights and obligations.
• Ensures understanding by family/significant others when the patient cannot.
• Develop individualized care plans in collaboration with patients and relevant professionals.
• Maintain accurate and up-to-date records of patient interactions, assessments, and service plans.
• Provide patients and families with information about services, resources, and strategies to manage conditions.
• Regularly monitor patient progress and adjust discharge plans as necessary.
• Work with multidisciplinary teams to ensure comprehensive care and support.
