Sarah Bush Lincoln Health Center is seeking a Case Manager to assess, plan, implement, and evaluate the needs of patients to ensure they receive appropriate services and support. This role collaborates with clinicians, social workers, and community resources to facilitate effective discharge planning and improve client outcomes. Hours: Monday–Friday, 8:00 a.m.–4:30 p.m.

Requirements: Active RN in Illinois. ADN required; BSN or BLS/CPR certification within 90 days of hire.

Hours: Monday-Friday, 8am-4:30pmRequirement: Active RN in the State of IL**Responsibilities**Advocate for patients’ needs and rights within the healthcare and social services systems., Conduct comprehensive assessments to determine patient needs, healthcare goals and eligibility for services., Coordinate services and resources, including referrals to healthcare providers, community resources, and support services., Delivers termination of benefits letters to patients as determined by third party payers, assuring patient understanding and awareness of rights and obligations.
