Job summary

Inova Mt. Vernon Hospital in Alexandria, VA is seeking an Experienced Registered Nurse Case Manager I to join the Case Management Team. This full-time role operates Monday – Friday with rotating weekends and focuses on discharge planning and coordination of care.

The RN Case Manager provides liaison between patients and the team, uses utilization management, ensures regulatory compliance and supports performance improvement initiatives, with focus on post-acute and acute settings.

Qualifications

• Certification: Basic Life Support (American Heart Association)
• Licensure: Registered Nurse in Virginia or multi-state license
• Experience: 1 year of case management or clinical care experience
• Education: BSN or ASN with plan to obtain BSN within 5 years

Responsibilities

• Provides discharge planning and continuity of care for acute and post-acute patients.
• Initiates and facilitates referrals to clinics, home health, hospice and other services.
• Collaborates with the interdisciplinary team and families to coordinate care transitions.
• Ensures timely regulatory compliance and facilitates precertification and payer authorization.
• Documents discharge planning information in medical records per standards.
