Job summary

Inova Health System is seeking an experienced Registered Nurse Case Manager 1 for our Medical Units. This full-time role entails weekday schedules with rotating weekends at Inova Mt. Vernon Hospital.

You will lead discharge planning and coordinate care across acute and post-acute settings, collaborating with the care team to optimize patient outcomes and manage utilization efficiently.

Qualifications

• Minimum of 1 year of case management and/or clinical care experience.
• Basic Life Support (American Heart Association) certification.
• Licensed or eligible for licensure in Virginia as a Registered Nurse or multi-state RN license.

Responsibilities

• Provide discharge planning and continuity of care for assigned patients in acute and post-acute settings.
• Coordinate services and act as liaison between patients, families and healthcare team.
• Utilize utilization management techniques to assess medical necessity and efficiency of care.
• Facilitate referrals to clinics, home health, hospice, SNF, LTAC and other services as needed.
• Collaborate with internal and external case managers and healthcare teams.
• Educate patients and families and address barriers to care progression.
• Document discharge planning in medical records and ensure regulatory compliance.
