Job summary

Inova Fairfax Hospital is seeking a dedicated experienced Registered Nurse Case Manager 3 to join the Case Management team. The RN Case Manager develops, implements and evaluates patient care plans and coordinates discharge planning across acute and post-acute settings.

You will collaborate with physicians and the care team to ensure timely, appropriate care, manage utilization and payer authorization, and actively participate in performance improvement initiatives.

Qualifications

• Basic Life Support (BLS) certification required.
• Accredited Case Manager (ACM), CCM, or MCG certification preferred.
• Licensed or eligible for Virginia RN licensure.

Responsibilities

• Develops, implements and evaluates patient care plans and progression throughout the continuum of care.
• Provides discharge planning and continuity of care for assigned patients in acute and post-acute settings.
• Initiates and facilitates referrals to clinics, home health care, hospice, SNF, acute rehab, LTAC and other services.
• Collaborates with the interdisciplinary healthcare team, patients and families in assessment and coordination of discharge planning needs.
• Documents discharge planning information in medical records according to standards and care plans.
• Monitors performance indicators and assists in driving practice improvements.
