Job summary

Inova Fairfax Hospital is seeking an Experienced Registered Nurse Case Manager 1 to join the Case Management Team. This full-time role runs Monday to Friday with rotating weekends and supports discharge planning and continuity of care in acute and post-acute settings.

The RN Case Manager 1 coordinates services, liaises with patients, families and the interdisciplinary team, and uses utilization management to ensure appropriate, efficient care.

Qualifications

• BSN or higher nursing degree and RN licensure in Virginia.
• 1+ year of case management or clinical care experience.
• Experience coordinating discharge planning and payer processes.
• Strong communication with patients, families and healthcare teams.

Responsibilities

• Provide discharge planning and continuity of care for assigned patients.
• Coordinate services and act as liaison between patients, families and the care team.
• Apply utilization management to determine medical necessity and efficiency of care.
• Facilitate precertification and payer authorization when indicated.
• Participate in clinical performance improvement initiatives.
• Educate patients and families and address barriers to care progression.
