Qualifications
- Certified in Basic Life Support (American Heart Association) and licensure as a Registered Nurse in Virginia or eligible for licensure
- 1 year of case management and/or clinical care experience
- Bachelor's Degree in Nursing or equivalent required; ADN with BSN plan within 5 years of start date
Responsibilities
- Provide discharge planning and continuity of care for assigned patients in acute and post-acute settings
- Coordinate services and act as liaison between patients, families and interdisciplinary healthcare team
- Utilize utilization management techniques to determine medical necessity and appropriateness of care
- Facilitate precertification and payer authorization processes as indicated
- Participate in clinical performance improvement activities
